Apache: [core:info] AH00128: File does not exist: /var/www/html/favicon.ico (404)
The requested file isn't in the DocumentRoot — a normal 404 entry in the Apache error log.

If the URL should work: check the file name case, DocumentRoot, deployment, and rewrites. For favicon.ico, robots.txt etc. it's just browsers/bots asking — add the file or ignore the entry. With frameworks, missing rewrite to index.php also produces this.
Common causes
- File missing or wrong case
- Wrong DocumentRoot
- Rewrite to front controller not working
- Bots probing common paths
- Stale links
⚡ Quick fix
- Create/deploy the file or fix the link
- Ensure
.htaccessrewrite to index.php works (AllowOverride All) - Ignore bot noise (or block with rules)
Detailed fix by platform
Linux
sudo grep AH00128 /var/log/apache2/error.log | awk '{print $NF}' | sort | uniq -c | sort -rn | head
