Windows lost access to the system drive during boot — storage driver/controller mode change, failed update, or disk/BCD problems.

Meaning
Switching SATA mode between AHCI/RAID/IDE in BIOS, enabling Intel VMD/RST, cloning to a new NVMe, or an update installing a storage driver can all leave Windows without the right boot driver.
Common causes
- BIOS storage mode changed (AHCI ↔ RAID/RST/VMD)
- Cloned/migrated system drive
- Bad update or pending updates
- Disk failure or file system corruption

- From Recovery: uninstall the latest update, run chkdsk and bootrec
- Boot Safe Mode once to load the AHCI driver
